New Life RTF Albany is seeking a dedicated and organized Outpatient Mental Health Case Manager to join our clinical care team. This role involves providing ongoing case management and care coordination to individuals participating in outpatient mental health services. You will work closely with clinicians, medical professionals, and housing teams to ensure each participant receives the comprehensive support they need.
Responsibilities include developing individualized service plans, facilitating access to community resources, scheduling appointments, documenting progress, and advocating for participants’ wellness goals. A successful candidate is detail-oriented, empathetic, and confident in navigating mental health systems. You must be able to maintain professional boundaries, work independently, and communicate effectively with both participants and colleagues.
Prior experience in behavioral health, social work, or case management is preferred. Bachelor’s degree in psychology, human services, or a related field is strongly encouraged. This is a meaningful opportunity to be a bridge between individuals and the care that helps them thrive.
